At the time of the injury, one of the two staff members present did not have a "Pending" or "Cleared" Personal Background Check. According to records (attendence, staffing schedule, and timesheets) reviewed by this licensor, there were two staff and 13 school age children on the playground at the time of the incident. Only one qualified Aide was with 13 children.
